One year of current, acute-care experience

Current CPR Card (must be American Heart)

PPD within one year or chest X-ray

Valid Driver's License

Social Security Card

Physical exam within one year

Valid Nursing License / Certificate

Additional Discipline Specific Certificate(s)

Titers and other health documents
